Who do I contact for a question on a bill for service?

The Mohegan Tribal Fire Department contracts with a third-party vendor for EMS billing.  The name of the company is Certified Ambulance Group and they can be reached at 800-336-6402.

How do I get a copy of my EMS patient care report?

If you are the patient:

You are entitled to a copy of your EMS report.  We must be able to verify your identity to protect your confidential medical information.  A release will need to be signed to comply with federal HIPAA regulations.  Please call 860-862-7333. for more information.  

Attorney for the patient:

You will need to provide a HIPAA-compliant release signed by the patient.  You can fax it to 860-862-7331 or call 860-862-7333 for more information.

Any other situations:

Please call 860-862-7333 to discuss.
